If you know someone who could benefit from practical, person-centred support, you can refer them here. We’ll respond promptly, confirm the right service pathway, and guide the next steps with care and clarity.

Referring someone should feel straightforward. Whether you’re a family member, support worker, health professional, school, hospital, or community service, you can submit a referral in just a few minutes.

Once we receive your referral, we’ll:

Review the details and confirm urgency and needs
Contact the participant (or nominated contact) to discuss goals and consent
Match the person with the most suitable support option
Explain timelines, availability, and what happens next

Refer to other programs

We carefully assess the needs of younger mums within our program to determine when referral to specialised services is necessary. Our experienced team actively listens to their concerns, ensuring that we understand the unique challenges they face. If a younger mum requires support beyond our program’s scope, we collaboratively connect them with various external organisations. This holistic approach ensures that each younger mum receives comprehensive assistance tailored to their specific circumstances.
